Who is the hospice care team?


Many people with a variety of skills work together for the benefit of the patient and loved ones, and hospice uses a team approach to coordinate care. If they qualify, patients can benefit from a hospice comfort care team comprised of a physician, RN, CNA, social worker, and chaplain. Volunteer and bereavement programs are also provided. A hospice nurse and practitioner are on call 24 hours, seven days of the week to provide support and care of pain or symptoms.

As time passes, what can we expect?


The time may come when the patient's needs require, or the family can no longer manage to provide, the level of care required around the clock. Volunteer assistance is available for a variety of services, including respite for the family, companionship, transportation, errands and meal preparation.

Hospice understands the changes and adjustments that life-limiting illness presents to families and patients.

What about bereavement support?


Grieving the loss of a loved one is a natural part of healing. Bereavement support assists family members by offering care and counseling to help them adjust to the loss of a loved one.
